Caring coalition

The Cupertino-based Chinese American Coalition for Compassionate Care (CACCC), an organization devoted to end-of-life issues for Chinese Americans, is collaborating with researcher Dr. Shu-Ju Yang, PhD, to improve end-of-life care for those living with serious illness in the Chinese community.

Yang is doing her project, titled “Advance Care Planning and Starting the Conversation Initiative,” through a Community Ambassador Fellowship program run by Cigna, a global health services company.

“End-of-life conversations and advance care planning can be extremely challenging, yet profoundly influential for patients and family members,” Yang said in a statement. “I want to help those in need be better prepared by directing their energy toward what truly matters and what is meaningful to them at life’s end. … CACCC’s expertise and resources will help make this important transformation possible.”

CACCC has planned a three-month virtual program that includes training volunteers; advance care planning, hospice and palliative care presentations and guest speakers who will share their respective palliative care experiences. The coalition will also conduct and facilitate Heart to Heart cafes, designed to engage the general public, patients, family members, caregivers and healthcare providers to share their end-of-life wishes using the CACCC’s Heart to Heart cards.
